The LTHHCP RN provides nursing services and care to Home
Care patients in their place of residence. Responsibilities include
providing assessments, care planning, and service coordination, along
with providing direct care to each patient and instruction/supervision
to HHAs, PCAs and other caregivers. Ideal incumbents will possess an
in-depth knowledge regarding current nursing treatment standards and
practices as well as a willingness to have a flexible schedule that
accommodates patient needs. . Previous work with adults is required,
with previous nursing experience in med-surge or a home care
setting strongly preferred.
JOB REQUIREMENTS:
Must possess a valid NYS Nursing license and
registration. Position requires current driver's license and reliable
transportation. Serves as the on-call RN as needed. Bi-lingual
(Spanish) required.
